Coach Feedback
We Want and Appreciate Your Feedback
Being a coach is a big responsibility and substantial time commitment, and we’re all tremendously grateful for the commitment your coach gave to AYSO, your kids, and the Six Philosophies this year.
When parents submit Coach Feedback, the league gains important insight not only about coaches, but also about your players’ experience this past season and Soccer Year.
This information is used for internal purposes only in evaluating future coach assignments and in evaluating and improving the coach training we provide.
Instructions For Completing Coach Feedback
Thank you for taking the time and following the steps below to provide your thoughtful feedback!
Log-in to the Team Website
Log-in to your Dashboard at our Team Website at ayso300team.org/app

Complete the "Coach Evaluation Form (Confidential)"
Read carefully the 1-5 scale and the 5 questions asked and provide answers for the questions.
Please note: Question #5 is only being asked generally in evaluation of your coach. We do not use the answer you give to this question in any way to form teams or make team assignments next year.
Optional: Enter "Additional Comments"
You may use this section for any additional comments about your coach and/or your player’s AYSO experience.
If you choose, add any additional context to the answers you gave to the 5 questions.
The Coach Feedback questions only relate to your team’s *Head Coach*, so the Additional Comments section could also be used to provide feedback on assistant coaches if you choose.
