WAIT LIST REGISTRATION WILL CLOSE 3/20/10
WAIT LIST REGISTRATIONS MUST BE RECEIVED VIA OUR P.O. BOX BEFORE A PLAYER CAN BE ELIGIBLE TO BE PLACED ON A TEAM. Please read the directions below for complete details to be on the wait list for AYSO soccer.
After completing the on-line portion of your registration, mail the items listed below to: AYSO 300, P.O. Box 74, Western Springs, IL 60558. If any of the items listed below are missing from your mail-in registration, it will be returned to you and marked incomplete making your child(ren) ineligible to be placed on a team. All checks will be held until player placement.
YOU NEED TO SUBMIT THE FOLLOWING
Signed and dated player registration forms
Signed and dated volunteer registration form
A valid copy of volunteer's drivers license or State issued ID
A birth certificate for any U5 player or any new player to AYSO 300
One check per player for $85 to cover fees.
